Senior Mental Health Clinician, Community Treatment & Transition Program
Opportunity for a Senior Clinician to join our Community Treatment & Transition team (Social Worker, Occupational Therapist, Psychologist or Nurse)

About the role:
This position is located at the Community Forensic Mental Health Service (CFMHS) in Clifton Hill, 15 minutes from Melbourne’s CBD. CFMHS is a state-wide agency that works with individuals who have a serious mental health illness and have offended or are at risk of offending. The Community Treatment and Transition (CTT) Program provides treatment and supervision for forensic patients transitioning from Thomas Embling Hospital (TEH) to ensure they are well supported through their reintegration back into the community. The program assists forensic patients (patients defined under the Crimes (Mental Impairment and Unfitness to be Tried) Act 1997 to meet the conditions of their Extended leave until they progress onto a non-custodial supervision order.
As a senior mental health clinician, you will work within a small multi-disciplinary team and oversee a case load of clients who are transitioning or have been discharged from Thomas Embling Hospital and are on Extended Leave. The role involves providing in reach support to patients and clinical teams in Thomas Embling Hospital via the provision of advice, consultations on discharge planning and legal processes.
